If you would like to request an appointment, please send a message to our office. One of our staff will then call you to schedule your appointment.

If you need to cancel an appointment, please notify our office by phone (402-637-0800) or send a message at least 48 hours in advance.

Patient Forms

Please fill out the patient forms (health history, demographic, and HIPAA compliance) and bring them with you to your appointment.

 

  • Completed pre-registration forms (available on the Patient Forms page)
  • Insurance card(s)
  • Photo ID (such as a driver's license)
  • Copay (for more information on copays, pdf click here )
  • Put together a list of prescriptions, allergies, surgeries, and individual and family health history (diabetes, heart disease, etc.)

 

If you don't have the opportunity to complete your pre-registration forms ahead of time, please arrive 30 minutes prior to your appointment time. If your pre-registration forms are complete, simply arrive 15 minutes early and give the paperwork to the front desk. If you've been to OrthoWest in the past and you have a new injury, you will still need to complete the registration forms, including a health history and possibly a questionnaire.

  • Check in with a receptionist when you arrive.
  • For your security, at your first appointment we will take your photo and verify with a photo id.
  • The receptionist will request your copay when you check in (this amount is listed on the front of most insurance cards). Many policies have a different copay amount for specialists (sp-copay) than for family physicians.
  • If your pre-registration forms are not complete you will be given the necessary paperwork to fill out.
  • When your exam is complete, check out/schedule your next appointment with the receptionist.

OrthoWest requires that workers' compensation claims be authorized by your employer's workers' compensation carrier prior to your visit. If not authorized, we will require a $170 payment, or you may reschedule after your claim is authorized.

If you did not inform us that this was a work injury when the appointment was made, please call (402) 637-0802 and we will attempt to authorize the visit prior to your scheduled appointment.
